The name of the applicant, father's name / mother's name, date of birth etc. should be spelt correctly in the form as it appears in the corporation birth certificate. No corrections will be allowed later on.

It is mandatory to fill all the fields marked ( * ) in red.In case a field is not applicable to you please mention NA (Not Applicable).

An incomplete registration form will be automatically rejected.

After completing the form, click on 'Submit' button you will receive an acknowledgement receipt with your USER ID & PASSWORD .

You will also receive your USER ID and PASSWORD through an SMS on your registered mobile.

You are requested to take a print of the duly filled registration form  

You are requested to retain a copy of the duly filled registration form for future correspondance.
